GP+ Nottingham City – extended hours access for patients
Offering Evening & Weekend Appointments bookable through your GP practice, patients registered at this practice are able to access additional routine appointments during evenings and weekends through the new GP+ service.
Appointments are available to see GPs, Practice Nurses, Clinical Pharmacists and Physiotherapists in a fully equipped accessible location on Upper Parliament Street in Nottingham City Centre.
Opening hours are:
16:00-20:00 Monday – Friday
9:00-13:00 Saturday & Sunday

MEDICAL INTEROPERABILITY GATEWAY
From time to time it is helpful for us to be able to share information about your health and care requirements with other health organisations that are responsible for providing you with healthcare. Across Nottinghamshire we are introducing a new system called MIG (Medical Interoperability Gateway) which will enable us to make relevant medical information available from your GP record with other healthcare professionals who are providing you with direct care. The MIG allows for relevant information to be viewed by other healthcare professionals, however before your information is accessed you will be asked for your explicit consent. Examples of organisations that may access your GP health information include NEMS Out of Hours team, walk in centres and local A&E departments.
Sharing of information in this way is designed to ensure that the healthcare professionals looking after you have the most relevant information to enable them to provide you with the most appropriate care. The type of information shared is restricted and includes a summary of current problems, current medication, allergies, recent tests, diagnosis, procedures, investigations, risks and warnings – all this information is currently held in your GP system record.
We have automatically set up the sharing facility to allow your information to be made available via the MIG. Whenever a clinician from another healthcare organisation wishes to view your GP information via the MIG they will always seek your permission before doing so; if you say “NO” they will not be able to see any information and no data will be made viewable via the MIG.
If you have any concerns about the sharing of your information or do not want your information being made available via the MIG please ask to speak to the Practice Manager who can advise accordingly.
All patients have a named GP attached to their record. If you would like to know your named GP, please ask at Reception. Please be aware this is for admin purposes only and you can see any GP of your choice, when available.

ON LINE ACCESS TO MEDICAL RECORDS
In addition to booking appointments and ordering medication patients will also be able to access their Summary Patient Record, on line, from 31 March 2015. Patients wishing to register for this service must complete an application form available from the surgery and supply photo ID. From the 31st March 2016 Patients will be able to register to see Coded information on their medical records
Additionally full GP2GP Electronic Transfer of Medical Records will commence at the practice in January 2015
If you would like a chaperone present during your consultation, please ask when booking your appointment, or when you arrive for your appointment, or when you enter your consultation. Our staff are fully trained to support you during your appointment
If you would like a language interpreter or a hearing languages interpreter, please ask at reception. We can also provide leaflets in other languages on request
If you require any vaccination relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.please call in to collect a Travel vacs form to be filled in and returned to the surgery prior to a appointment being given
