The Practice will be closed on Bank Holiday Monday 28th August 2017.  We will open again on Tuesday 29th August at 8am.

The Practice will be closed on Tuesday 12th September 2017 from 12.00 for staff protected learning.  We will open again at 8.00am on Wednesday 13th September 2017.

When the practice is closed please telephone 111 to be directed to our out of hours services all calls from landlines and Mobiles are free or phone 999 in a life threatening emergency. We apologise for any inconvenience.

New Urgent Care Centre - Oct 15

URGENT CARE CENTRE

The Clifton Nurse Access Point closes on 28 September 2015. Treatment Room and Phlebotomy Services will continue to be provided at Clifton Cornerstone.

In October 2015 the new NHS Urgent Care Centre will open at Seaton House, London Road, Nottingham NG2 4LA (next to the BBC building).

The centre will offer assessment and treatment for health conditions that are urgent but non-life threatening such as:

Minor burns and scalds

Minor head injury with no loss of consciousness

Skin infections and animal bites

Suspected broken bones, sprains and strains (X-ray will be available on-site)

Eye infections and minor eye injuries

No appointment is needed; just drop in between 7am and 9pm 365 days a year

The new NHS Urgent Care Centre will replace the existing NHS walk-in services:

Walk-in service at the Health Centre, Upper Parliament Street

o

The walk-in service will end 30th September.

o

The Health Centre premises will remain open to provide health services to patients registered with the Platform One Practice

Clifton Nurse Access Point, Clifton Cornerstone

o

The walk-in service will end on 28th September

o

Treatment Room (regular wound dressing service) and phlebotomy (blood testing) will continue to be provided from Clifton Cornerstone

Walk-in Centre, Seaton House, London Road

o

The current walk-in service will end on 30th September

o

The new NHS Urgent Care Centre will open from the same location on 1st October

MIG 

MEDICAL INTEROPERABILITY GATEWAY

From time to time it is helpful for us to be able to share information about your health and care requirements with other health organisations that are responsible for providing you with healthcare. Across Nottinghamshire we are introducing a new system called MIG (Medical Interoperability Gateway) which will enable us to make relevant medical information available from your GP record with other healthcare professionals who are providing you with direct care. The MIG allows for relevant information to be viewed by other healthcare professionals, however before your information is accessed you will be asked for your explicit consent. Examples of organisations that may access your GP health information include NEMS Out of Hours team, walk in centres and local A&E departments.

Sharing of information in this way is designed to ensure that the healthcare professionals looking after you have the most relevant information to enable them to provide you with the most appropriate care. The type of information shared is restricted and includes a summary of current problems, current medication, allergies, recent tests, diagnosis, procedures, investigations, risks and warnings – all this information is currently held in your GP system record.

We have automatically set up the sharing facility to allow your information to be made available via the MIG. Whenever a clinician from another healthcare organisation wishes to view your GP information via the MIG they will always seek your permission before doing so; if you say “NO” they will not be able to see any information and no data will be made viewable via the MIG.

If you have any concerns about the sharing of your information or do not want your information being made available via the MIG please ask to speak to Marion Tongue or Caroline Carter who can advise accordingly.

BLOOD TESTS

With effect from 3 November 2014, our patients will be able to choose a provider for this service. As we continue to undertake this work at the practice, and in view of the convenience, we will assume that patients will want to use John Ryle Medical Practice unless we are advised otherwise 

ON LINE ACCESS TO MEDICAL RECORDS.

In addition to booking appointments and ordering medication  patients will also be able to access their Summary Patient Record, on line, from 31 March 2015. Patients wishing to register for this service must complete an application form available from the surgery and supply photo ID. From the 31st March 2016 Patients will be able to register to see Coded information on their medical records

Additionally full GP2GP Electronic Transfer of Medical Records will commence at the practice in January 2015

If you require any vaccination relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements.please call in to collect a Travel vacs form to be filled in and returned to the surgery prior to a appointment being given

We make every effort to give the best service possible to everyone who attends our practice

However we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint if this is so we would wish for the matter to be settled as quickly,and as amicably as possible.
